Free discovery Call

Step 2

After the free discovery call, we will schedule and order your comprehensive labs to evaluate your overall health and create a customized service plan to help meet your goals. The cost of our initial lab panel is $175.00. (All subsequent follow-up lab draws are included in monthly subscription costs.)

Step 3

After your labs are completed, we will schedule you for an initial comprehensive consult with our provider to go through each lab result and to work together to create a custom plan for you. This consultation for all subscriptions costs $100. However, we will credit that $100 towards your initial monthly fee when you sign up for a monthly subscription plan.

All subscriptions include follow up visits, labs, medications and supplies!

Step 4

Follow-up visits and labs will be scheduled every 3 months at first then every 6 months when levels are stable.

There are no annual provider fees and no additional lab fees while enrolled in a subscription plan. This is a savings of $300- $700 when compared to other providers.

*Our weight-loss clients will have weekly call options and monthly visits include in their plan.*

Revitalize Health and Wellness LLC offers all inclusive pricing with no hidden fees or charges. The monthly subscription rates are the areas first all inclusive packages. Many competitors will have base rates that do not include consult fees and follow-up labs! We take pride in being affordable and promoting the safest hormone treatments in Wisconsin!

- All follow-up labs, unlimited consults/ access to providers, shipping, supplies and program medications are included!

-For our men's health clients, this means plans that include aromatase inhibitors and selective estrogen receptor modulators when warranted without extra charges!

2. Safe and Comprehensive Care!

We are focused on helping you discover the best version of you without cutting corners. Our medications, including hormones, are on an automatic refill every 4 to 6 weeks! Even if the previous vial has medication remaining, the best and safest practice is to discard any remaining medication and start fresh every month! We follow FDA regulations and guidelines to help eliminate risks of infection and reduce complications. Many competitors will require the use of 1 vial of medication to be used over a period of 1-3 months while continuing to BILL for a subscription every 4 weeks!

3. Access to Treatment Providers!

Your treatment team consists of a local provider who is dedicated to seeing you succeed! Throughout your journey with Revitalize Health and Wellness LLC, you will have direct contact and visits with your treating provider. Lab review, consults and medication questions are handled by your dedicated team. Don't be tricked by clinics offering guidance through support staff or "treatment questioners." Have your questioned answered and guided by medical clinicians every step of the way!
